Custom facial implant surgery is planned individually. Smaller or more straightforward cases may be performed in Face Clinic’s own surgical setting, usually as outpatient surgery when clinically appropriate.
Larger, more complex or combined procedures — such as full facial matrix implants or custom implants combined with facial plastic surgery, including rhinoplasty, facelift or other facial procedures — are organised in a hospital environment.

3D planning is the digital process used to study the patient’s facial bone structure before designing a custom facial implant. It allows Dr Riba and the engineering team to analyse the anatomy, identify the areas that need projection or support, and design an implant adapted to the patient’s facial skeleton.
The implant is planned before surgery, rather than adjusted during the procedure, so the design can be reviewed, refined and manufactured specifically for that patient.
3D planning is important because custom facial implants must fit the patient’s anatomy precisely. It helps assess facial proportions, asymmetries, bone contours and the relationship between different areas of the face before surgery.
This process supports a more controlled surgical plan, especially in cases involving the chin, jawline, cheekbones, infraorbital area, facial asymmetries or more complex custom facial matrix designs. Patient-specific planning and CAD/CAM workflows are used in maxillofacial surgery to improve anatomical fit and preoperative preparation.
Custom facial implants are most commonly planned for areas such as the chin, jawline and cheekbones. However, 3D planning can also be used to assess other facial regions when there is a clear clinical indication, such as the infraorbital area, midface support, nasolabial region, forehead depressions, asymmetries, post-traumatic defects or selected reconstructive needs.
The final indication depends on the patient’s anatomy, tissue quality, aesthetic goals, medical history and the safety of the surgical plan.
No. Not every patient needs a custom 3D facial implant. Some patients may be better suited to a standard implant, another surgical approach, a non-surgical treatment or no treatment at all.
A custom implant is considered when the patient needs a more precise, patient-specific structural solution, especially in cases of asymmetry, complex facial contouring, previous surgery, reconstructive needs or when standard implants are unlikely to provide the desired anatomical fit.
