Do Cheek Implants Lift the Face or Jowls?
Cheek implants can create a more lifted appearance in selected patients, but they do not lift the face in the same way as a facelift. Their main role is to improve cheekbone projection, midface support and facial structure.
Do cheek implants lift the face?
Cheek implants can make the face look more lifted when the main issue is flat cheekbones, poor midface projection or loss of structural support. By improving the contour of the cheekbones, they can give the midface a firmer, more defined and more youthful appearance.
However, cheek implants do not physically lift sagging skin. They do not remove excess skin, reposition descended soft tissues or correct significant jowls. Their effect is structural: they add projection and support to the cheek area.
The distinction is important. Cheek implants restore volume and facial framework; a facelift repositions soft tissues.
How cheek implants can improve facial support
Cheek implants are placed over the cheekbone area to enhance the underlying facial skeleton. Depending on the patient’s anatomy and implant design, they can improve:
- Flat or under-projected cheekbones.
- Midface hollowing.
- A tired or sunken facial appearance.
- Poor facial definition.
- Lack of balance between the cheeks, chin and jawline.
- Reduced structural support in the central face.
In patients with weak cheekbone projection, the overlying soft tissues may lack support. By restoring projection at the skeletal level, cheek implants can make the midface look more defined and better supported.
Do cheek implants lift jowls?
Cheek implants may have a limited indirect effect on very mild lower-face heaviness, but they are not a treatment for established jowls.
Jowls usually develop because of soft tissue descent, skin laxity, changes in facial fat and age-related loss of support. Cheek implants can improve the midface, but they do not directly reposition the tissues along the jawline.
If jowls are mild and the patient also has flat cheekbones or midface deficiency, cheek implants may improve the overall balance of the face. However, if there is significant sagging, a facelift, deep plane facelift or cervicofacial lift may be more appropriate.
Do cheek implants lift the lower face?
No, not directly. Cheek implants are designed to improve the cheeks and midface, not the lower face.
They can improve the transition between the lower eyelid, cheek and central face, but they do not correct loose skin along the jawline, heavy jowls, neck laxity or significant lower-face sagging.
In some patients, better cheek projection can make the entire face appear more harmonious. But this is a visual and structural improvement, not a true lower-face lift.
Do cheek implants make you look younger?
Cheek implants can create a younger appearance when facial ageing is mainly related to poor midface support, flat cheekbones or loss of facial structure.
They may be useful when the patient has:
- Midface volume loss.
- Flattened cheek contours.
- Under-eye hollowing associated with poor cheek support.
- Loss of facial definition.
- A tired appearance despite reasonable skin quality.
However, if the ageing appearance is mainly caused by loose skin, deep folds, jowls or neck laxity, cheek implants alone will usually not be enough.
Cheek implants vs facelift: what is the difference?
Cheek implants and facelift surgery treat different anatomical problems.
Cheek implants
Cheek implants are mainly used to improve bone structure, facial projection, cheekbone definition and midface contour. They are especially relevant for patients with naturally flat cheekbones, malar deficiency, facial imbalance or age-related loss of support in the midface.
Facelift or deep plane facelift
A facelift is designed to improve soft tissue descent, jowls, lower-face sagging and jawline laxity. When neck ageing is also present, it may be combined with a neck lift or cervicofacial lift.
A deep plane facelift works on deeper facial layers and can be particularly useful when facial ageing is caused by tissue descent rather than volume loss alone.
Are cheek implants better than a facelift?
Neither procedure is automatically better. They solve different problems.
Cheek implants may be more suitable when the main concern is flat cheekbones, poor midface projection, facial imbalance or lack of definition.
A facelift may be more suitable when the main concern is jowls, loose skin, lower-face sagging, neck laxity or descent of deeper facial tissues.
In some patients, the most complete result may require both approaches: structural enhancement with facial implants and soft tissue repositioning with facelift surgery.
Can cheek implants and facelift surgery be combined?
Yes. In selected cases, cheek implants can be combined with facelift surgery, neck lift surgery or deep plane facelift.
This combination can be useful when there are two different issues at the same time:
- Lack of facial structure or projection, especially in the cheekbones, chin or jawline.
- Soft tissue descent or laxity, especially in the lower face or neck.
In Dr Francisco Riba’s practice, facial implants are often assessed together with facial rejuvenation procedures. In some cases, cheek implants may be combined with a facelift or cervicofacial lift to improve both skeletal support and soft tissue position.
This can also apply to chin implants or jaw implants, particularly when the objective is not only to improve facial projection, but also to achieve a more refined and balanced rejuvenation result.
Dr Riba is a specialist in oral and maxillofacial surgery, facial implant surgery and advanced facial rejuvenation procedures, including deep plane lifting. This allows treatment planning to consider both the facial skeleton and the ageing of the soft tissues.
When cheek implants alone may be enough
Cheek implants alone may be appropriate when the patient has:
- Good skin quality.
- Mild or no jowling.
- Flat or under-projected cheekbones.
- Lack of midface definition.
- Facial imbalance caused mainly by poor bone projection.
- Realistic expectations about what implants can and cannot do.
In these cases, cheek implants can improve facial contour without necessarily requiring a facelift.
When a facelift may be needed instead
A facelift, deep plane facelift or cervicofacial lift may be more appropriate when the patient has:
- Moderate or severe jowls.
- Loose skin in the lower face.
- Neck sagging.
- Deep folds caused by tissue descent.
- Significant ageing changes beyond volume loss.
- A heavy lower face that does not come mainly from weak cheek projection.
If the main concern is facial sagging, cheek implants alone may not meet the patient’s expectations. If the concern is flat cheeks and lack of midface support, cheek implants may be more relevant.
Why specialist assessment matters
The same visual complaint can have different anatomical causes.
Two patients may both feel that their face looks tired or lower than before. One may have poor cheekbone projection and midface hollowing. Another may have good bone structure but significant soft tissue laxity.
Their treatment plans should not be the same.
A specialist assessment should evaluate bone structure, facial proportions, midface projection, skin quality, soft tissue descent, jawline definition, neck laxity, previous fillers or surgery and patient expectations.
Key message
Cheek implants can make the face look more lifted by improving cheekbone projection and midface support. But they do not physically lift sagging skin, remove excess tissue or replace a facelift.
For mild midface deficiency, cheek implants may be enough. For significant jowls, lower-face sagging or neck laxity, facelift surgery, deep plane lifting or cervicofacial lifting may be necessary.
In many cases, the most refined result comes from combining structural facial implants with facial rejuvenation surgery when both bone support and soft tissue descent need to be addressed.
Frequently asked questions
Do cheek implants lift the face?
Cheek implants can create a lifted appearance by improving cheekbone projection and midface support. However, they do not lift the face in the same way as a facelift. They add structure; they do not reposition loose skin or descended soft tissues.
Will cheek implants lift jowls?
Usually, no. Cheek implants may slightly improve the appearance of very mild lower-face heaviness in selected patients, but they are not a treatment for established jowls. Significant jowling usually requires a facelift, deep plane facelift or cervicofacial lift.
Do cheek implants lift the lower face?
Cheek implants do not directly lift the lower face. They are designed to improve the midface and cheekbone area. If the lower face is sagging, the patient may need a lifting procedure rather than cheek implants alone.
Do cheek implants make you look younger?
They can make the face look younger when the ageing appearance is caused by flat cheekbones, poor midface support or volume loss. If ageing is mainly due to loose skin or jowls, cheek implants alone may not be sufficient.
Are cheek implants better than a facelift?
Cheek implants are not better or worse than a facelift. They treat a different problem. Cheek implants improve structure and projection; a facelift repositions sagging soft tissues. Some patients may benefit from one procedure, while others may need both.
Can cheek implants be combined with a facelift?
Yes. Cheek implants can be combined with facelift, neck lift or deep plane facelift surgery when the patient has both lack of facial projection and soft tissue sagging. This combined approach can improve facial structure and rejuvenation at the same time.
How do I know if I need cheek implants or a facelift?
A specialist assessment is needed. If the main issue is flat cheekbones or poor midface projection, cheek implants may be appropriate. If the main issue is jowls, loose skin or neck sagging, a facelift or cervicofacial lift may be more suitable.
